Just checking to see if you needed more help. I am running K2 on all my sites now and using RT templates.
Yes. I had to re-install K2. It was one of the customizations that I had made. I don't remember which modification caused the problem, but it was one that I found on these forums that worked in other templates.

Does the image upload work when you try to upload a very small image? If so, try increasing your max_upload and memory_limit in your php.ini.
Does this happen in both front-end and back-end editing?
Does this blank window only open inside the lightbox or does the whole screen go white?

What happens when you try to upload an image? Do you get an error? A white screen? Does it tell you that you were successful, but not show the image? Is there no button to even add an image?
Share some more details.
Was this ever resolved? I have the same problem. I actually have to close the window and then re-open it. Not an acceptable solution.

Don't forget to increase your memory_limit. Mine is 62 mb, but you may want to go a bit higher if your max upload is 150.

Do you use Firebug? It is a FireFox plugin that shows you exactly which code you need to change, and it will even let you change it to test. If you don't already use it, take a look.

Whatever is done will modify the core, but will work.
How about putting the category id's in a field like the xtra filelds and teach the core to parse them out.
Its changing the way things work - not likely to happen unless a module like followeyes…
I don't think the concept of the single-category per item content structure is a way of thinking that is advanced and, therefore, a reason to change the content construction strategy and our subsequent way of thinking. It is merely a huge limitation…

you can get the extra fields like this if it suits your needs
$custom = $this->item->extra_fields;
and then addressing extra fields for the item like
echo $custom[1]->value; depending on the position, 0, 1,2,3,4 etc in the array
